The referral coordination program is designed to assist PCN physicians with referrals to specialist services. The referral coordinator will receive referral letters from the PCN physician and ensure the referral is processed appropriately and efficiently.  A specialist database will be established to maintain an inventory of specialists, programs/services, wait times, referral requirements etc.  The referral will be sent to the specialist and the referring physician will be notified and updated on the status of the referral.  Patients will be notified with appointment dates/times and information will be shared as appropriate.
